Maybe the simpler solution is to move to a system where the utilities are always responsible for repairing the sidewalk to the original state within x days, subjected to acceptance by the city for the work performed. The city should not be responsible for initiating and managing repairs ever for utility cuts.

The old system was that the City did the repairs (with the utility paying). This enabled them to repair cuts made by several utilities in same block , e.g. Hydro, Enbridge and Water, at the same time. More recently they have started to allow utilities to pay less and make their own repairs and some (Toronto Water as an example) are quite fast but others (e.g. Rogers) are not. This is new construction so there is no excuses for this to happen over and over, it seems to happen on most projects.
Toronto Hydro is owned by the city, IMO there is no excuse for this, the city has the power to make sure the required services are in place before completion and not after completion.
I travel to many cities in Asia and Europe and i don't recall seeing this.
This even happened at The Well after the stone was installed.
Good point but, in this case I THINK these utility cuts have nothing specific to do with this building but are part of a Hydro rebuild of conduits in the area - there are also cuts on Lombard, Adelaide and Richmond made at the same time.
